Insurance for SIIT/TU Students
How
to Claim Compensation
(1) Case of dismemberment and loss
of sight, permanent disability and medical expenses (each
accident): |

Insured
students, beneficiaries or student's guardian (s) proceeds : |

1)
Notify the insurance company or the University about the accident
immediately or as soon as possible.

2) Submit
the required documents listed below to the Student Affairs Section
of Thammasat University within 180 days of the accident.

1. Completed
Claim Form of Compensation for the injury
2. Medical
evidence, such as a certificate of diagnosis, certificate of medical
treatment result, X-ray film and so on.
3. Receipt
for medical expenses (original issue only)
4. Case
of dismemberment: a photo of student's full appearance is required.
5. A
photocopy of student's citizen ID card and a photocopy of student's
Insurance Card
6. Case
of permanent disability:
- If insured student becomes a quasi-incompetent person, court
order to appoint the curators is required.
- If insured student becomes an incompetent person, court order
to appoint the patrons is required.
- Medical evidences certify or identify that insured students
loss of organs, loss of sight or permanent disability
7. Letter
of guarantee by Thammasat University
8. Other
documents (if available)
Remark: The insurance company will
not pay compensation money for any medical evidences

(2) Case of loss of life caused by
accident : |
|
Procedure: |
Beneficiaries,
student's guardian(s), relatives, or heir-at-law |
|
|
1)
Notify the insurance company or the University about the accident
immediately or as soon as possible.
2) Submit
the required documents listed below to the Student Affairs Section
of Thammasat University within 30 days of the loss of life.

1. Certificate
of death
2. Two photocopies of record of the accident by police officer.
(In the case for loss of life caused by accident only)
3. Two photocopies of autopsy performance by hospital doctor.
(In the case for loss of life caused by accident only)
4. Two photocopies of insured student's house registration and
beneficiaries' house registration
5. Two photocopies of insured student's citizen ID card and beneficiaries'
citizen ID card
6. Two photocopies of student's insurance card, power of attorney,
and statement
7. If the beneficiary is a juvenile, the legal guardian appointment
by the court is required.
8. Letter of guarantee by Thammasat University
9. Two photocopies of beneficiaries' marriage certificate or divorce
certificate. (if available)
10. Other documents (if available)
Remark: Additional required document is a letter of guarantee
issued by Thammasat University for loss of life caused by accident
in the area or boundary of Thammasat University or places possessed
by Thammasat University (or participation in activity in/out campus). |
